Referrals, results & medical records
Good records help explain what has already been assessed. Ask reception which information is needed and how it should reach the practice.
Ask about a referral
If your GP or another specialist suggested a consultation, mention that when booking. Ask whether a referral letter is required and how it should be supplied. If you already have one, keep it with the information requested for your appointment. This guide does not create a universal referral rule.
Bring dates as well as results
Previous blood tests and imaging can be useful when a clinician is comparing findings over time. Ask which reports and images are relevant. A date, laboratory reference range and the circumstances of a test may be important; a number without its report can be incomplete.
Keep medication information current
Prepare a list of prescribed medicines and relevant supplements, with the details you normally provide to a clinician. Include recent changes and previous treatment where relevant. This page does not recommend stopping medicine or changing a dose before the visit.
Use the agreed communication channel
Call reception for instructions about sending records. Do not assume the website has a secure upload or patient portal; this demo has neither. The contact page provides practice enquiry details, while reception can explain the appropriate route for your documents.
